Bloom Bakers Club — Parent Document Package
Little 8 Bloom LLC, a Georgia limited liability company, doing business as Little 8 Bloom Baker Club ("the Club", "we", "us").
This packet contains every form a parent or legal guardian must review, complete and sign before a child participates in the Bloom Bakers Club. Print it, complete it, and bring it to your baker's first meeting or email it to angel@little8bloom.com.
Legal notice: This document package is provided as a starting template for Club families and is not legal advice. Georgia law limits what may lawfully be waived, and no waiver excuses gross negligence, reckless conduct or intentional wrongdoing (see O.C.G.A. § 1-3-7).

1. Parent/Guardian Membership Agreement
Download this formThis Agreement is entered into between Little 8 Bloom LLC, a Georgia limited liability company, doing business as Little 8 Bloom Baker Club, and the undersigned parent or legal guardian.

Membership term and fees
- One-time new baker joining fee: $35.00, due at enrollment. Includes the Bloom Baker Welcome Kit.
- Monthly member dues: $20.00 per baker, due on the 1st of each month.
- Membership continues month to month until the parent cancels in writing.
- Fees are paid through the Club's PayPal links. A $10 fee may apply to returned or failed payments.
- Dues cover ingredients, decorating supplies, badges, donation drives and meeting materials.
Meeting schedule
- Chapters meet on the 1st and 3rd Sunday of each month.
- Each chapter is limited to eight (8) bakers.
- Meeting location, start and end times are provided in writing before each session.
- Schedule changes, weather closures and make-up dates are communicated by email and text.
Attendance and pickup requirements
- Bakers must be signed in and signed out by an authorized adult at every meeting.
- A parent, guardian or authorized adult must remain reachable by phone for the entire meeting.
- Children are released only to adults listed on the Authorized Pickup and Transportation Form, with photo ID.
- Late pickup fees apply as stated in Section 5.
Refund and cancellation policy
- The $35 joining fee is non-refundable once the welcome kit has been ordered or issued.
- Monthly dues are non-refundable for the current month once the first meeting of that month has occurred.
- Cancel by emailing angel@little8bloom.com at least seven (7) days before the next billing date to stop future dues.
- Missed meetings are not refunded or prorated; make-up sessions are offered when available.
- If the Club cancels a meeting and cannot reschedule, a prorated credit is applied to the next month.
Club rules and expected behavior
- Bakers and parents agree to the Parent and Child Code of Conduct in Section 6.
- All kitchen safety instructions from Club staff must be followed immediately and completely.
- Closed-toe shoes, tied-back hair and clean hands are required in the kitchen.
Permission to participate in baking activities
- The parent grants permission for the child to participate in supervised baking, mixing, rolling, cutting, decorating, packaging, simulated cookie sales and community donation activities.
- Activities involve the use of ovens, hot pans and trays, mixers, small hand tools and food ingredients.
Right to suspend or terminate membership
- The Club may suspend or terminate a membership at any time for unsafe behavior, harassment or bullying, repeated non-payment, repeated late pickup, or conduct by a parent or child that disrupts the Club.
- Where termination is for cause, unused prepaid dues for future months are refunded at the Club's discretion.
Electronic-signature consent
- The parent agrees that an electronic signature, typed name, or checkbox submitted through the Club's website or email has the same legal effect as a handwritten signature under the Georgia Uniform Electronic Transactions Act and the federal E-SIGN Act.
- The parent may request a paper copy of any signed document at any time.

2. Assumption of Risk and Liability Waiver
Download this formI understand that participation in Club baking activities involves inherent risks that cannot be eliminated regardless of the care taken by the Club.
Foreseeable risks I acknowledge
- Hot ovens, pans, trays, cooling racks and freshly baked goods
- Stand mixers, hand mixers and other kitchen equipment
- Knives, spatulas, scrapers, cutters and decorating tools
- Slips, trips, falls, spills and broken glass or ceramics
- Flour dust inhalation and exposure to raw dough and eggs
- Burns, cuts, bruises, choking, foodborne illness and allergic reactions, including severe reactions
Assumption of risk and release
- I have read the risks above, I understand them, and I voluntarily authorize my child's participation.
- To the fullest extent permitted by Georgia law, I release and hold harmless Little 8 Bloom LLC, its members, staff, volunteers and venue hosts from claims for ordinary negligence arising out of my child's participation.
- This release does not apply to gross negligence, reckless or willful misconduct, or intentional wrongdoing, and does not waive any right that may not lawfully be waived under Georgia law, including O.C.G.A. § 1-3-7.
- I agree that this waiver is governed by Georgia law and that venue for any dispute lies in the county where the Club operates.

3. Allergy and Medical Information Form

Allergen warning
- Club meetings use wheat flour, butter, milk, eggs and sugar, and may involve nuts, peanuts, soy, coconut and other allergens brought in by families or present at the venue.
- Little 8 Bloom cannot guarantee an allergen-free or cross-contact-free environment. Shared surfaces, tools, ovens and airborne flour dust make cross-contact possible.
- Parents of children with severe allergies must supply any needed medication and confirm the child is able to self-manage or that an authorized adult will attend.

4. Emergency Medical Authorization
Download this formIn the event of illness or injury during a Club activity, and when I cannot be reached, I authorize designated Club staff to:
Authorizations granted
- Contact 911 and emergency medical services
- Administer or assist with parent-authorized emergency medication listed on the Allergy and Medical Information Form, where Club policy permits
- Arrange for emergency examination, treatment or transport by qualified medical personnel when I cannot be reached
- Provide responders with relevant medical, allergy and medication information for my child
Financial responsibility
- The parent or legal guardian is responsible for all costs of emergency transport, examination and treatment, including any amount not covered by insurance.
- This authorization does not replace first-aid training, supervision or insurance, and does not obligate Club staff to provide medical care beyond basic first aid.

5. Authorized Pickup and Transportation Form

Pickup rules
- Photo ID is required at every pickup until staff know the adult by sight.
- Children are never released to anyone not listed on this form. Additions must be made in writing by the parent.
- Late pickup: a $1 per minute fee begins 15 minutes after the posted end time. Repeated late pickups may end membership.
- If a child is not picked up within 60 minutes and no contact can be reached, staff must contact local authorities.
- Emergency pickup: staff will call emergency contacts in the order listed on the Allergy and Medical Information Form.
Transportation
- The Club does not provide transportation to or from regular meetings unless stated in writing.
- A separate field-trip and transportation consent form is required any time bakers leave the regular meeting location.

6. Parent and Child Code of Conduct

- Use kind, respectful language with staff, guests and fellow bakers
- Follow every kitchen and safety instruction the first time it is given
- Never bully, tease, exclude or harass another baker, in person or online
- Wear closed-toe shoes and appropriate clothing, with long hair tied back
- Wash hands on arrival, after breaks, and whenever instructed, and follow food-safety rules
- Take no photos or video of other children without staff permission
Parents agree to
- Model respectful behavior at meetings, in group chats and on social media
- Deliver and collect bakers on time and communicate absences in advance
- Raise concerns privately with Club leadership rather than in front of children
- Not photograph or post images of other families' children without their consent
Discipline
- Step 1: verbal redirection. Step 2: brief break from the activity. Step 3: parent contact.
- Step 4: suspension from one or more meetings. Step 5: removal from the Club.
- Unsafe behavior, violence, or harassment may result in immediate removal without prior steps.
- The parent remains the legal contracting party regardless of whether the child also signs.

7. Privacy Notice and Online Registration Consent

- Parent contact details, child's first name and age, chapter choice, birth month and year, allergy and medical information, emergency contacts and pickup authorizations — used to operate the Club safely.
- Payment records — used to process dues and joining fees.
- Website analytics — collected only if you accept analytics cookies in our cookie banner.
Who can access it
- Club leadership and designated staff or volunteers with a need to know.
- Service providers we use to run the Club: our website and database host, Shopify (merchandise orders), PayPal (payments), our email delivery provider, and Google Sheets/Analytics.
- We do not sell personal information or share it for third-party advertising.
Retention and your rights
- Membership records are retained while the child is enrolled and for up to three years afterward, or longer where required by law or insurance.
- Parents may request access to, correction of, or deletion of their family's information by emailing angel@little8bloom.com.
- We respond to verified requests within 30 days.
Children's privacy (COPPA)
- Registration must be completed by a parent or legal guardian, not by the child. Children may not create accounts or submit their own information.
- COPPA generally requires verifiable parental consent before a child-directed service collects personal information from children under 13. By signing this package, the parent provides that consent for the information described above.
- A parent may withdraw consent and request deletion of the child's information at any time; the child's membership may end as a result.
